Nature of Service

Flow-Sole is a wellness service providing foot reflexology, massage, gua sha, and relaxation techniques. This service is provided by KeoniJ (Ivan Keoni Johnson), a licensed massage therapist operating under Flow Recovery (DBA), Honolulu, Hawaii. Flow-Sole sessions are not a substitute for medical treatment, diagnosis, or advice from a licensed physician or healthcare provider.

Not Medical Treatment

Reflexology and massage are complementary wellness practices. Nothing communicated during or in connection with your session constitutes medical advice. If you have a medical condition, you are advised to consult your physician before receiving this service. The therapist reserves the right to decline or modify service if, in their professional judgment, proceeding is contraindicated for the client's safety.

Voluntary Participation

I confirm that I am voluntarily choosing to receive this session. I understand the nature of the service as described above. I have accurately disclosed all relevant health information in this intake form. If my health status changes between now and my session, I agree to notify the therapist before we begin.

Informed Consent — Gua Sha

Gua sha scraping is a traditional technique that involves applying pressure with a smooth-edged tool to the skin. It may produce temporary redness or petechiae (small red marks) on the skin surface. These marks are a normal physiological response and typically resolve within 24–72 hours. By signing this form, you confirm that you understand and consent to this technique being used as part of your session.

Release of Liability

To the fullest extent permitted by Hawaii law, I voluntarily release and hold harmless KeoniJ (Ivan Keoni Johnson) and Flow Recovery from any claim, liability, loss, damage, or expense arising from or related to this session, including but not limited to aggravation of pre-existing conditions, unforeseen reactions to techniques or products, or injury resulting from undisclosed health conditions. This release does not apply to claims arising from gross negligence or willful misconduct.

Privacy

Your personal and health information collected in this form is used solely to provide your session safely and is not shared with third parties. You may request deletion of your intake information at any time by contacting KeoniJ directly.

Cancellation

Please provide at least 24 hours notice to cancel or reschedule your session. Late cancellations or no-shows may be subject to a cancellation fee at the therapist's discretion.

Age Requirement

Clients must be 18 years of age or older to receive service without a parent or legal guardian present and co-signing this form. By signing below, you confirm you are 18 or older, or that a parent/guardian has authorized your participation.
